Background Implementation science presents ethical issues not well addressed by traditional research ethics frameworks. There is little empirical work examining how clinicians whose work is affected by implementation studies view these issues. Accordingly, we interviewed clinicians working at sites participating in an implementation study seeking to improve patient handoffs to the intensive care unit (ICU).Methods We performed semi-structured interviews with 32 clinicians working at sites participating in an implementation study aiming to improve patient handoffs from the operating room to the (...) ICU. We analyzed the interviews using an iterative coding process following a conventional content analysis approach.Results Clinicians’ greatest concern about involvement was possible damage to interpersonal relations with more senior clinicians. They were divided about whether informed consent from clinicians was necessary but were satisfied with the study’s approach of sending out mass communications about the study. They did not think opting out of the implementation portion of the study was feasible but saw this inability to opt out as unproblematic because they equated the study with routine quality improvement. Those clinicians who helped launch the study at their sites recounted several different ways of doing so beyond simply facilitating access.Conclusions The risks that clinicians identified stemmed more from their general status as employees than their specific work as clinicians. Implementation researchers should be attuned to the ethical ramifications of involving employees of varying ranks. Implementation researchers using hybrid designs should also be sensitive to the possibility that practitioners affected by a study will equate it with quality improvement and overlook its research component. Finally, the interactions that go into facilitating an implementation study are more various than the “gatekeeping” typically discussed by research ethicists. More research is needed on the ethics of the myriad interactions that are involved in making implementation studies happen. (shrink)

Food images are useful stimuli for the study of cognitive processes as well as eating behavior. To enhance rigor and reproducibility in task-based research, it is advantageous to have stimulus sets that are publicly available and well characterized. Food Folio by Columbia Center for Eating Disorders is a publicly available set of 138 images of Western food items. The set was developed for the study of eating disorders, particularly for use in tasks that capture eating behavior characteristic of these illnesses. (...) It contains foods that are typically eaten, as well as those typically avoided, by individuals with eating disorders. Each image has now been rated across 17 different attributes by a large general United States population sample via Amazon’s Mechanical Turk. Ratings included subjective attributes as well as estimates of nutrient content. Each participant rated a subset of stimulus set food items on all 17 dimensions. Additional description of the image set is provided in terms of physical image information and accurate nutritional information. Correlations between subjective ratings were calculated and an exploratory factor analysis and exploratory cluster analysis completed. Outcomes of the factor analysis suggested foods may be described along three latent factors of healthiness, tastiness, and umami taste; the cluster analysis highlighted five distinct clusters of foods varying on these same dimensions. Descriptive outcomes indicated that the stimulus set includes a range of foods that vary along multiple dimensions and thus is likely to be useful in addressing various research questions surrounding eating behavior and cognition in healthy populations, as well as in those with eating disorders. The provision of comprehensive descriptive information allows for stimulus selection that is optimized for a given research question and promotes strong inference. (shrink)

Persuasive technologies for health‐related behaviour change give rise to ethical concerns. As of yet, no study has explicitly attended to ethical concerns arising with the design and use of these technologies for vulnerable people. This is striking because these technologies are designed to help people change their attitudes or behaviours, which is particularly valuable for vulnerable people. Vulnerability is a complex concept that is both an ontological condition of our humanity and highly context‐specific. Using the Mackenzie, Rogers and Dodds’ taxonomy (...) of vulnerability, this paper identifies (a) the wrongs or harms to which a person is vulnerable, (b) the source of this vulnerability, and (c) the safeguards needed in response. Two ethical concerns with the design of persuasive technology for vulnerable people are discussed: the concerns of taking into account users' interests and their autonomy. (shrink)

Patients with psychiatric illness feel the brunt of the intersection of many of our society's and our health care system's disparities, and the vulnerability of this population during the Covid‐19 pandemic cannot be overstated. Patients with psychiatric illness often suffer from the stigma of mental illness and receive poor medical care. Many patients with severe and persistent mental illness face additional barriers, including poverty, marginal housing, and food insecurity. Patients who require psychiatric hospitalization now face the risk of transmission of (...) Covid‐19 due to the inherent difficulties of social distancing within a psychiatric hospital. Patients whose freedom and self‐determination have been temporarily overruled as they receive involuntary psychiatric treatment deserve a setting that maintains their health and safety. While tele‐mental health has been rapidly expanded to provide new ways to access psychiatric treatment, some patients may have limitations in technological literacy or access to devices. The social isolation, economic fallout, and potential traumatization related to the current pandemic will disproportionately affect this vulnerable population, and society's duties to them must be considered. Isaiah Berlin and Stuart Hampshire's early engagements with logical positivism and ordinary language philosophy are examined as historical and philosophical reference points for locating an alternative – interpretive and humanist – tradition that developed within analytic philosophy at Oxford in the 20th C. Berlin and Hampshire's writings show the legacy of an enduring Idealist philosophy, one that nonetheless had to be revised and reinvented against the new empiricist challenges brought on by the rise of analytic philosophy. Berlin and Hampshire rejected (...) idealism's metaphysical pretensions of the Absolute in favor of the new empiricism's insistence on grounding philosophy in experience, but staunchly opposed applying the latter's narrowly ‘scientistic’ view of knowledge to human experience, re-affirming the indivisible connections between epistemological issues and moral and political issues. The idealist themes they expounded are most clearly evident in their arguments for an interpretive philosophy in opposition to the reductivist tendencies of logical positivism, and in their defense of humanist liberalism against the drive of analysis toward naturalism where inquiry into human life is concerned. Such themes include: (i) an anti-naturalist, vitalist, philosophy of human sciences, (ii) an insistence on the intrinsic force and importance of human values against moral relativism, and (iii) the recognition of the political significance of the plurality of human values. As such, Berlin and Hampshire reveal the strong interpretive and humanist ways of reasoning from within the analytic tradition itself. Moreover, these interpretive and humanist themes continue to have strong echoes, this paper argues, in the development of post-analytic political theory in the latter half of the 20th century through today, as further evinced in the ideas of Bernard Williams (1929–2003) and Charles Taylor (b. 1931). By calling attention to such continuities, this paper reveals how moral and political philosophy in the Anglophone world lay not moribund but continued to develop in the heyday of analytic philosophy from the late ‘30s to the ‘50s and onward, thereby challenging the commonplace of the ‘death’ of normative political theorizing until Rawls reinvigorated it in the ‘70s. ☆ Special thanks are owed to Amy McCready, an astute reader of a preliminary draft of this paper. Wittgenstein formulates the paradox of gestalt switches thus: ‘What is incomprehensible is that nothing, and yet everything has changed, after all. That is the only way to put it’. In the course of isolating what I take to be the best of the various solutions to the paradox explored by Wittgenstein, the following claims are defended: (a) A significant strand in Wittgenstein’s own formulation of, and solution to, the paradox can best be understood as a response to three specific claims (...) made by the Gestalt psychologist Kohler. (b) The most promising avenue Wittgenstein explored in his many attempts to resolve the paradox gives perceptual attention a constitutive role in the solution (c) This role is best elaborated, partially, by appeal to information processing theories of attention. (d) There are good reasons to think that the kind of solution to the paradox this yields would have been welcomed by Wittgenstein. (shrink)

Racial essentialism or the idea of unchanging racial substances that support human social hierarchy, was introduced into philosophy by David Hume and expanded upon by Immanuel Kant. These strong influences continued into W. E. B. Du Bois’ moral and spiritual idea of a black race, as a destiny to be fulfilled past a world of racism and inequality. In the twenty-first century, »the race debates« between »eliminativists« and »retentionists« swirl around the lack of independent biological scientific foundation for physical human (...) races and the ongoing importance of race as a social ordering principle and source of identity. Analyses of the idea of race are of philosophical concern for historical and conceptual reasons, as well as ongoing issues of contemporary identity and social injustice. (shrink)

In the summer of 1941, Harald Sverdrup, the Norwegian-born Director of the Scripps Institution of Oceanography (SIO) in La Jolla, California, was denied security clearance to work on Navy-sponsored research in underwater acoustics applied to anti-submarine warfare. The clearance denial embarrassed the world renown oceanographer and Arctic explorer, who repeatedly offered his services to the U.S. government only to see scientists of far lesser reputation called upon to aid the war effort. The official story of Sverdrup's denial was the risk (...) of blackmail over relatives in occupied Norway. Declassified documents tell a different story. Although Sverdrup's integrity was defended on the highest levels of U.S. science, doubt was cast upon him by members of his own institution, who accused him of being a Nazi sympathiser. Personal distrust, rooted in scientific and intellectual disagreement, spilled over into questions about Sverdrup's loyalty and judgement. These doubts were considered sufficient grounds for withholding clearance, until Roger Revelle, a former student of Sverdrup now working within the Navy, was able to obtain a limited clearance for Sverdrup to develop techniques to forecast surf conditions during amphibious assaults. After the war, this work was credited with saving many lives, but at the time it placed Sverdrup out of the mainstream of Navy-sponsored oceanographic research. In being denied access to major areas of scientific work, Sverdrup's position as a leader of American oceanography was undermined.The loyalty case of Harald Sverdrup illustrates the emergence of an institutional apparatus through which the U.S. military began to control and shape the organisation of American science in the twentieth century. Military sponsorship of scientific research, begun during the open conflicts of World War II and continuing into the simmering tensions of the Cold War, involved explicit control by the U.S. military of who had access to critical information. This in turn meant who could do science in conjunction with the military. As the U.S. Navy became the principal sponsor of oceanography in the post-war years, clearance to do military work became to a great extent clearance to do oceanography. Choices about who could be trusted were also choices about who would do science, and what kind of science they would do. (shrink)

On the basis of research in the social identity tradition, we contend (a) that identification and differentiation are not mutually exclusive, (b) that a sequence in which identification gives way to differentiation is not necessarily associated with superior organizational outcomes, and (c) that social identification, and leadership that builds this, is generally a prerequisite for group success.

Background: There is limited research on brain lesions in children with cerebral visual impairment of heterogeneous etiologies and according to associated subtyping and vision dysfunctions. This study was part of a larger project establishing data-driven subtypes of childhood CVI according to visual dysfunctions. Currently there is no consensus in relation to assessment, diagnosis and classification of CVI and more information about brain lesions may be of potential diagnostic value.Aim: This study aimed to investigate overall patterns of brain lesions and associations (...) with level of visual dysfunction and to compare the patterns between the classification subgroups in children with CVI.Methods: School-aged children with CVI received ophthalmological and neuro-psychological/developmental assessments to establish CVI-related subtyping. Other pediatric information was collected from medical records. MRI scans were coded according to a semi-quantitative template including brain regions and summed for total scores. Non-parametric analyses were conducted.Results: 28 children had clinical brain MRI scans available [44% of total sample, Group A n = 16, Group B n = 12]. Total brain scores ranged between 0 and 18 and were widespread across regions. 71 per cent had post-geniculate visual pathway damage. The median total brain and hemisphere scores of Group B were higher than subgroup A but differences did not reach statistical significance. No statistically significant associations were found between brain scores and vision variables.Conclusion: This study found a spread of lesions across all regions on the brain scans in children with congenital CVI. The majority had damage in the postgeniculate visual pathways and visual cortex region suggesting this is an area of interest and potentially informative for diagnosis. However the subtyping classification did not show differences in number or region of lesions though the trend was higher toward Group B. This study confirms the complex diffuse and variable nature of brain lesions in children with congenital CVI, many of whom have other neurological impairments. (shrink)

Despite changes in their representation and visibility, there are still serious concerns about the inclusion and day-to-day workplace challenges various groups face (e.g., women, ethnic and cultural minorities, LGBTQ+, people as they age, and those dealing with physical or mental disabilities). Men are also underrepresented in specific work fields, in particular those in HEED (Health care, Elementary Education and the Domestic sphere). Previous literature has shown that group stereotypes play an important role in maintaining these inequalities. We outline how insights (...) from research on stigma, social-identity, and self-regulation together increase our understanding of how targets are affected by and regulate negative stereotypes in the workplace. This approach starts from the basis that members of negatively-stereotyped groups are not just passive recipients of negative attitudes, stereotypes, and behaviors, but are active individuals pursuing multiple goals, such as goals for belonging and achievement. We argue that it is only by understanding stigma from the target’s perspective (e.g., how targets are affected and respond) that we can successfully address workplace inequality. Key in this understanding is that stereotypes, prejudice and discrimination have taken on much more subtle forms, with consequences for the way members of stigmatized-groups cope. These insights lead us to propose an approach to understanding barriers to workplace equality that highlights four key aspects: a) the different (often subtle) potential triggers of identity-threat in the workplace for members of stigmatized-groups, b) the ways in which members of stigmatized-groups cope with these threats, c) the role of supportive factors that mitigate potential threats and affect self-regulation, and d) potential hidden costs for the self or others of what appears at first to be effective self-regulation. The focus on threats, coping, support, and potential hidden costs helps us understand why current diversity efforts are not always successful in increasing and maintaining members of stigmatized-groups in organizations, and provides insight into how we can aid efforts to effectively lower barriers to workplace equality. The Continuum Encyclopedia of British Philosophy" employs a wide construal of 'philosophy' that was common in former centuries. Its biographical entries include writers on mainstream philosophical topics whose individual contribution was small (for example, writers of textbooks or minor critics of major figures). But the encyclopedia also includes celebrated figures from other intellectual domains (e.g. poets, mathematicians, scientists and clergymen), who had something to say on topics that count as broadly philosophical. This interdisciplinary approach, coupled with sophisticated indexing and cross-referencing, (...) makes "CEBP" easily accessible to students and specialists across a huge range of subjects. It will become a highly important resource to support teaching and research in literature, history, classics, cultural studies, politics, economics, language, aesthetics, psychology and science, as well as in mainstream philosophy itself. In conveying experiences of meditation, the question of what exceeds or should resist description has been a recurrent topic of commentary in a wide array of literature -- including religious doctrine, meditation guides, and contextual accounts written by historians and social scientists. Yet, to date, this question has not significantly informed neuroscientific studies on the effects of meditation on brain and behaviour, in large part -- but not wholly -- because of the disregard for first-person accounts of experience that still (...) characterizes neuroscience in general. By juxtaposing perspectives from non-neuroscientific accounts on the tensions and questions raised by what is and is not expressed or expressible in words, this article paves the way for a new set of possibilities in experimental contemplative neuroscience. What is human reason, how did it arise, how is it connected to animal reason? In The Enigma of Reason (ER) Hugo Mercier and Dan Sperber (2017) suggest that reason evolved driven by the need to support communication and coordination in small human groups. They contrast this to the idea that the function of reason is to enable humans to make better decisions and develop more accurate beliefs. After a summation of the ER argument, the theory is critiqued and two (...) of their assumptions -- that human cognition is based on massive modularity and that reason co-evolved with and is dependent upon language -- are challenged. Alternatives to these assumptions are presented from the perspective of pattern matching processes in neural dynamical systems, which it is asserted provides a more realistic approach to the functioning of the brain and mind. At the moment in Britain and elsewhere the debate inside and outside of Parliament on various medical issues which are essentially moral never ends. Everybody has his own point of view--or principles. But what emerges for society to adopt can often be called in lay terminology 'compromise'. Professor Mitchell argues in this paper that a moral consensus is possible and indeed ought to be achieved, as today the medical practitioner can no longer make his decision only in accordance with the (...) strict code of ethics of the medical profession. Current thinking on the development of molecular microbial characterisation techniques in public health focuses mainly on operational issues that need to be resolved before incorporation into daily practice can take place. Notwithstanding the importance of these operational challenges, it is also essential to formulate conditions under which such microbial characterisation methods can be used from an ethical perspective. The potential ability of molecular techniques to show relational patterns between individuals with more certainty brings a new sense of urgency to already (...) difficult ethical issues associated with privacy, consent and a moral obligation to avoid spreading a disease. It is therefore important that professionals reflect on the ethical implications of using these techniques in outbreak management, in order to be able to formulate the conditions under which they may be applied in public health practice. This case for discussion highlights some of the ethical difficulties that may arise in the use of molecular typing techniques in the control of infectious diseases. Molecular typing techniques offer evidence (stronger than regular epidemiological exploration of sources and contacts) for claims about infection routes. Such evidence will mean that public health authorities need to think about how to respond ethically to causal responsibility for contagion.
